Medicare's Annual Wellness Visit
How to Make the Most of your Free Benefit
It's rare to get anything for free in healthcare, but if you're a Medicare recipient, the Annual Wellness Visit is one of those valuable exceptions.
Unlike a traditional physical exam, this completely covered yearly appointment focuses on prevention and planning. It's an opportunity to take stock of your current health, identify risks for future problems, and build a personalized plan with your doctor to help you stay healthy and independent.
And just so you know, ShadowNurse is Medicare eligible and can walk you through not just how to use this benefit, but also how best to follow the personalized health plan your doctor draws up for you.
Are You Eligible?
The first question you'll want answered is whether you truly qualify for the benefit. The short answer is that, yes, most Medicare beneficiaries qualify. That said, there are a few things to know.
If you've been enrolled in Medicare Part B for a year or more, you are eligible for the Annual Wellness Visit. After your first visit, you can schedule another Annual Wellness Visit every 12 months at no cost. That means no copay, no coinsurance, and no deductible, as long as your doctor accepts Medicare patients. Check before you go.
Patients who are new to Medicare sometimes confuse their "Welcome to Medicare" visit with the Annual Wellness Visit. They are different. The welcome visit, also known as the Initial Prevention Physical Examination, is a one-time visit scheduled within your first 12 months of enrollment. The Annual Wellness Visit is the yearly visit after your first 12 months in the program.
What This Visit Really Is (and Isn't)
The Annual Wellness Visit isn't the doctor's visit you're more familiar with, where you go for a very specific ailment. These visits don't include taking your vitals, such as listening to your heart or ordering bloodwork. These are your chance to have a conversation with your doctor and for your doctor to check in with you. Instead of focusing on specific symptoms, it's a chance to get an understanding of your overall health.
During these annual visits, your doctor will review your health history. In addition, your doctor will assess your risk factors and help identify gradual changes that can slip under the radar. Sometimes, little things like slight changes in your weight, blood pressure, or blood sugar can indicate something more. Catching symptoms early could help prevent them from becoming bigger problems in the future.
It's also worth noting that if you bring up a new symptom or concern during your Annual Wellness Visit, your provider may address it, but that portion of the visit may be billed separately.
To keep the visit fully covered, it helps to stay focused on the wellness and prevention agenda.
How to Prepare: What to Bring
To make the most of your Annual Wellness Visit, a little preparation goes a long way. You should note any issues that pop up in your own wellness journal so you don't forget.
You want to get as much information from the doctor about your current health as possible. So right before your appointment, gather the following:
A complete list of all medications, including prescriptions, over-the-counter drugs, vitamins, and supplements. Include the dosages, too.
Your Medicare card and any supplemental insurance information.
Notes on your family medical history, especially heart disease, diabetes, cancer, or other hereditary conditions.
A list of questions or health concerns you'd like to discuss may help you remember issues you've had in the past 12 months.
Any recent test results or records from specialists you've seen.
What Happens During the Visit
All the information you bring in will come in handy, because during the Annual Wellness Visit, your doctor will cover specific areas, like your health history, a health risk assessment, preventive screenings if needed, and a grab bag of other issues.
Family health history is a cornerstone of anyone's health record. However, during this visit, your doctor will want to learn not just about your and your family's health history, but also about any medications and supplements you use. The medication and supplement information is important so that the doctor can review for any potential interactions or duplications.
Your family medical history is important because many conditions run in families, and keeping this information up to date helps your doctor tailor screenings and recommendations to your unique background.
Along with the health history, the doctor will also conduct a risk assessment. This often requires you to complete a short questionnaire that looks beyond clinical measurements and instead provides a more complete picture of your day-to-day life.
The questionnaire will cover topics such as your living situation and your access to nutritious food. The doctor will want to know whether you're maintaining social connections and if you are physically active. All of these factors contribute not just to your physical health but also to your emotional health. These are all data points that are often just as important as your lab results.
Along with the questionnaire, your doctor will review whether you are due for any upcoming preventive tests, such as a mammogram, colonoscopy, or bone density scans. If so, your doctor will refer you for one of those tests, if you haven't already had it scheduled.
Finally, the Annual Wellness Visit is also a time when your doctor will likely want to discuss whether you have an advance care plan, fall prevention strategies, mental health screenings, or cognitive health issues that need to be addressed. It's much easier to have these conversations before a health crisis, so use the annual visit to get this planning done.
After the Visit
After your annual visit, you'll get a personalized prevention plan. These are simple summaries of your health status, risk factors, and recommended next steps. But the value of that plan depends entirely on whether you use it, and this is where ShadowNurse can help, if you need it.
Follow-up appointments need to be scheduled. Referrals need to be acted on. The recommended screenings happen. For many Medicare patients, this is where things fall through the cracks—not from lack of effort, but because navigating the healthcare system is genuinely complicated.
This is exactly where ShadowNurse comes in. ShadowNurse's healthcare navigation service helps Medicare patients understand their benefits, prepare for appointments such as the Annual Wellness Visit, and, importantly, follow the steps their care plan calls for. We're now enrolling Medicare patients. If you or a loved one could use a knowledgeable guide in your corner, we'd love to hear from you.
