If you are a enrolled in Medicare, have at least one qualifying chronic condition (like high blood pressure, cancer, heart disease, COPD, diabetes, stroke, etc.), and get a written recommendation for healthcare navigation from your primary provider (typically your doctor or nurse practitioner), then you should qualify for covered services offered by ShadowNurse. Start by submitting your information via the form on this page and we’ll help you get set up from there.

To qualify for Medicare coverage of healthcare navigation services with ShadowNurse, you will probably need to get a written recommendation for healthcare navigation from your primary provider (typically your doctor or nurse practitioner). Once you are signed up on our waitlist form, we’ll help you get the approval that you need—this should be very straight forward if you have a qualifying chronic condition (like high blood pressure, cancer, heart disease, COPD, diabetes, stroke, etc.).

If you have Medicare, we don’t anticipate that you will have to pay anything out of pocket for the healthcare navigation services provided through ShadowNurse. In certain scenarios, Medicare enrollee’s may be required to contribute to a small portion of vendor’s fees. Sign up on our Medicare Waitlist Form and we’ll touch base with you to figure out if one of those scenarios applies to you.

Medicare (CMS) implemented a new January 2024 rule for Principle Illness Navigation (PIN) and other related services (Community Health Integration - CHI) and assessments (Social Determinants of Health - SDOH). Once you’ve joined our Medicare Waitlist, we’ll work with you to quickly assess your qualifications, complete the necessary steps, and pair you with one of our registered nurse navigators.
